Mandatory Sexual Assault Prevention Program

Students who have completed the 2011-12 Sexual Assault Prevention Training but did not turn in their proof of completion will no longer be able to access last year's module to print proof of completion. Please contact the Office of Admission at ug-admissions@csu.edu or 773-995-2513 to have your completion confirmed and registration hold removed. For students who began, but did not complete, last year's training program, you will need to complete the 2012-13 training module. Upon completion of the training, to expedite the release of your registration hold you can copy and paste onto an e-mail and forward it to: ug-admissions@csu.edu or send it via fax to 773-995-3820.  Or, if you wish, you can drop it off in the Office of Admissions, Cook Administration Building, Room 126.
